When responding to the City's Purchasing Department with a proposal please be sure to include the following information:
Company Name
Company Address (delivery and mailing address w/street, city, state, zip code)
Company Phone number(s)
Company Fax number(s) (this is where we will send Purchase Orders)
Contact Person name (including an e-mail address is desired)
Description and sub-description of product or service (examples: Excavator, building contractor, architect, civil engineer, environmental consultant, building demolition, pipe provider- concrete and acrylic), industrial cleaners, etc.
Completed documents can be hand-delivered to the City Hall, Faxed (269-329-4506) or mailed to:
